About Midlands AccessHealth:

Midlands AccessHealth is a partnership of community organizations that want to make health care accessible to those that do not have health insurance and cannot afford care. It serves as a medical home network providing access to primary care, pharmaceuticals, specialty care, and hospital care, behavioral health, disease management, and education for low-income, uninsured Richland County residents. Created in 2001 as Palmetto AccessHealth, the partnership’s services also include care management activities and supportive services for participants with hypertension and/or diabetes and demand management capability for participants who utilize the emergency departments. Health education and wellness/health promotion services are available to all participants.

Midlands AccessHealth has facilitated care for over 18,500 participants and continues to actively work on providing care for the county’s population in need. In 2010, RC was selected as a participant of AccessHealth SC, a program of the South Carolina Hospital Association with funding from The Duke Endowment. Partners participated in strategic planning activities to improve the network’s efficiency and effectiveness.
