About AccessHealth Tri-County Network:
The AccessHealth Tri-County Network (AHTN) was developed in 2010, with support from AccessHealth SC and The Duke Endowment, and is comprised of 27 partners providing a wide range of healthcare and health-related services that work to provide uninsured residents in the Tri-County region with a coordinated approach to access to healthcare. The Network partners consist of hospitals, free clinics, healthcare centers, outreach providers, Department of Mental Health, local health department and volunteer primary care/specialty care providers.
The mission of AccessHealth Tri-County Network is to coordinate a sustainable provider network of care for low-income, uninsured residents in Berkeley, Charleston and Dorchester counties.
The objectives of the network are focused on appropriate client referral, navigation to a medical home and specialists’ services and use of health information technology that allows for shared health information and electronic health records. The Network’s clients and partners voluntarily participate in the program.
