QUALIFICATIONS: |
· Bachelor’s degree required, Master’s degree preferred
· Experience in leading teams, collaboration, performance improvement, project planning and coordination
· Experience with philanthropy and grant management
· Proven ability to effectively work with multiple projects and groups
· Excellent communication skills
· Outstanding proficiency with Microsoft Word, Excel and PowerPoint is required
· Minimum 10 years of experience in community health, public health, nonprofit, healthcare, or related field. |

BASIC FUNCTION: |
The Director leads Community Health, Innovation and CaroNova initiatives under the direction of the VP, Innovation/SC CaroNova State Director and in partnership with a variety of stakeholders. This role is responsible for the design and implementation of grant programs, CaroNova initiatives and other efforts by overseeing technical assistance, data collection, evaluation and performance improvement efforts. This role also supports SCHA member hospitals in innovation efforts through alignment and partnership internally and externally to help members meet goals. |

DUTIES AND RESPONSIBILITIES: |

- Leverages program data and alignment with SCHA and CaroNova to analyze and use data and reporting to plan and enhance programs.
- Identifies, establishes, and develops partnerships internally and externally to fund and support SCHA and CaroNova programmatic goals and SCHA’s strategic plan in alignment with other stakeholders.
- Ensures grant program deliverables are met for Technical Assistance (coaching calls, peer learning opportunities, action plans, grant reviews and community capacity building) to program participants.
- Completes progress, financial, and other grant reports.
- Supports leadership in assisting with program design and the development of a library of technical assistance partners with targeted expertise
- Drafts partner contracts.
- Serves on CaroNova Action Teams to support strategy development and alignment with state-level resources
- Supports leadership in the development of staffing models and coordinate TA contracts to ensure program deliverables are met
- Communicates program effectiveness through producing and sharing meaningful reports, materials, and presentations.
- Fosters a highly collaborative environment in which program stakeholders are encouraged to engage in program objectives and identify and share best practices among programs and across states.
- Facilitates coordination, when applicable, with NC Healthcare Foundation technical assistance team and other contracted partners.
