Qualifications
- Bachelor’s degree required; master’s degree preferred
- Minimum 5 years of related experience; clinical experience preferred
- Maternal & Child Health experience preferred
- Excellent communication skills
- Proficient at Microsoft products
Basic Function
The Manager, Community Health & Social Drivers of Health, serves as an implementer in a Collective Impact initiative focused on the development of state and local infrastructure and pilot projects involving bundled payment to build statewide capacity in social drivers of health (SDoH). This position collaborates with multiple perspectives, recommends collective improvements, works with others to pilot and implement innovative models that improve outcomes, and drives ongoing progress.
Duties
- Identify, engage, and maintain relationships across multiple stakeholder groups and their related initiatives and develop and continue a common agenda
- Work with teams to align, co-design, implement, and assess workplans, timelines, and efforts
- Collaborate with partners to identify community readiness for implementation of social determinant of health innovations
- Utilize a collective impact approach and equity-informed decision-making to pilot innovations related to social determinants of health at the regional and local levels
- Among providers and other partners, gain consensus of proposed actions, develop opportunities for alignment, and drive meaningful action
- Understand current and emerging best practices related to areas of focus such as maternal and infant outcomes
- Drive implementation, spread, and alignment of social determinants of health innovations, practices, and processes
- Identify and facilitate access to relevant population-level data with partners to inform decision making and support strategic goals
- Work with partners to proactively produce and share relevant information to include reports, materials, and presentations
- Create, lead, and carry-out educational opportunities for hospitals and other partners
- Develop the scope of a technical assistance model to build local capacity and spread
