SCHA Launches “Using Data to Drive Action” CMS Equity Measures Workgroup
SCHA is excited to announce a new “Using Data to Drive Action” CMS Equity Measures Workgroup, with the goal of easing and ensuring hospital compliance with three new CMS equity measures: Hospital Commitment to Health Equity, Screening for Social Drivers of Health, and Screen Positive Rate for Social Drivers of Health. Over the course of four sessions, hospitals will have the opportunities to learn directly from CMS and the American Hospital Association (AHA), review their hospital-specific equity-related discharge data, share best practices with each other, and engage with other hospital leaders. Prework:
Prior to session one, hospitals will be asked to establish multi-disciplinary teams that are endorsed and approved by hospital leadership, watch a short video, review the CMS measures, and review hospital-specific inpatient and emergency department discharge data (this will be provided individually in advance of the first session).

SCHA asks to be invited to hospital board and executive meetings to discuss the hospital’s data and work throughout the workgroup.
