Delirium Prevention and the New CMS Age-Friendly Measures from Prisma Health Oconee Memorial Hospital

Delirium is one of the most common hospital-acquired conditions in older adults, yet it is highly preventable through evidence-based strategies such as early recognition, reorientation, mobility, and sensory support. With the introduction of new CMS Age-Friendly guidelines, hospitals are expected to align care practices with the “4Ms” framework—What Matters, Medication, Mentation, and Mobility.
