Integrating Advance Care Planning In Clinical Practices
The Problem:
Most clinicians are not trained in having important conversations regarding end of life with their patients, especially those with serious illnesses.
The Goal:
Agape Foundation participated in the My Life My Choices program, an advance care planning initiative created by the South Carolina Society for the Care of the Seriously Ill with support from BlueCross BlueShield of SC Foundation. Agape’s goal was to educate their staff on advance care planning to increase the number of these conversations and ensure their quality.
Method & Results:
Agape has trained over 218 health care providers, including 85 clinicians throughout South Carolina, who have collectively had 19,084 quality advance care planning conversations with patients since May 2018.
