Social Determinants Collaborative Webinar Series

The Collaborative continues to assist health systems in utilizing the Medicaid Quality-Based Directed Payments Program. This webinar will continue to explore the ICD-10 “Z” codes that allow hospitals to report social needs documented by physicians as well as case managers, discharge planners, social workers, and nurses. Utilizing these codes will allow hospitals and health systems to better track patient needs and identify solutions to improve the health of their communities.
