Social Determinants Collaborative Webinar Series

This is the 2nd webinar in our quarterly Social Determinants Collaborative Webinar Series.

Approved in February of 2018, the “Z codes” in the ICD-10-CM allows hospitals to report social needs documented by physicians as well as case managers, discharge planners, social workers, and nurses. Utilizing these codes will allow hospitals and health systems to better track patient needs and identify solutions to improve the health of their communities.
