Introduction to POST (South Carolina Physician Orders for Scope of Treatment) Virtual Workshop

This 1.5-hour introductory workshop will equip physicians and other health care professionals with the knowledge and resources needed to begin utilizing the POST form in South Carolina.  Participants will learn the purpose of the National POLST program, how it is being implemented as “POST” in South Carolina and why it is important for our citizens.  They will learn patient criteria for determining if the patient is a candidate for POST, resources regarding when and how to have the conversation with patients/families prior to initiating the form, and how to complete the form with the patient.

Other topics include: who can have the conversation, who can complete the POST form, process for reviewing, modifying, or voiding the form, and the eRegistry for storage of all advance care planning documents.  The eRegistry includes the “Goals of Care” form which is based on Ariadne Labs’ Serious Illness Conversation Program. A special segment on the use of POST within the context of COVID-19 patients is included to assist the practitioner in addressing the needs of this population of potentially seriously ill patients.

The webinar offers 1.50 nursing contact hour or 1 CME. It will also be offered again on September 23, October 21 and November 18.
